Job Position: Social Worker
Location: Bakersfield, CA 93301
Pay Rate: $31.19/hr
Shift: DAY
Classification: Non-exempt Category: Full time
Major Duties and Responsibilities:
Assist GPD and SSVF Bridge Case Managers and Program Directors in assessing the veteran s needs;
Develop, monitor, evaluate and implement short-term treatment plans with the veteran household in conjunction with the VA GPD Liaison (GPD Program only) or other agencies as appropriate;
Provide information and referral to other services available from government agencies and community based organizations;
Explain requirements and procedures for public assistance, act as an intermediary with other service providers and advocate for and help veterans get resources that would improve their well-being;
Assist veterans in filling out applications for assistance programs;
Work with a veterans family on reunification or other support as it relates to their treatment plan;
Assist veterans with a mental health or substance use diagnosis on a treatment plan and appropriate support programs;
Transport veterans to scheduled appointments if clinically necessary;
Conduct in-home visits with veterans in their residence to assess suitability of living conditions, evaluate the presence and level of risk in the household, and report findings to proper authority;
Respond to crisis situations;
Provide individual and group therapy;
Interpret social service rules, regulations and policies to veterans and their household;
Administer drug tests (witness urine samples given by clients);
Provide casework consultation and professional development to staff members without advanced training;
Maintain records, input data and retrieve information from case management computer systems;
Oversee field placements of interns in graduate programs;
Perform other job related duties as required.
Skills/Requirements:
Completion of a Master s degree in Social Work from an accredited college or university, which includes an internship;
Experience compiling and maintaining data records;
Experience with handling a caseload, developing service/treatment plans, monitoring compliance through home calls, field calls and other personal contact; modification of treatment plans as needed/required, authority to recommend sanctions for noncompliance impacting program participation;
Initiative, flexibility, self-motivated and capacity to respond effectively in stressful situations;
